Name and Address Requirements
To update your address or phone number, you may request that your record be updated by phone, e-mail or letter. Please Contact Us.
If you have changed your name, submit a letter of request to Teacher Development and Certification with documentation as below. A new certificate will be issued in your new name. There is no fee for this service.
Applicants married or who have legally changed their name in Canada must provide:
- A photocopy of marriage certificate(s), or if applicable, vital statistics name change certificate.
If the above documentation is not available the following is required:
- A statement sworn before a Commissioner for Oaths or Notary Public outlining all names used for legal purposes.
Applicants married or who have legally changed their name outside of Canada must provide:
- A statement sworn before a Commissioner for Oaths or Notary Public outlining all names used for legal purposes, or;
- Statutory declaration sworn before a Commissioner for Oaths or Notary Public and true copies of such documents attached, or;
- Original documents of legal names used.
Note: Documentation in a language other than English or French must be accompanied by an English or French translation along with the required statutory declaration form. Statutory declarations are not required if your translation was completed by an official translator.
Original documents can be submitted in person or by mail. They will be returned by registered mail.
